Referral Information

An authorized referral is required for most HMO insurance plans.  The referral must be initiated by the primary care provider's office listed on the insurance plan and it must be made to one of our providers:

The PCP office needs to obtain the authorized referral and it needs to be faxed to our office at (972)739-3304. 

The referral must authorize at least 12 visits and it must state the authorization time period of at least 6 months with beginning and ending dates.  This is to ensure that our providers are able to provide follow-up care for anything that needs to be done.
